In order for paperwork to be considered complete, you will need to fill out the following forms:

•      Enrollment Application: a one-page "reservation" with the appropriate application fee

•      Enrollment Form: a more complete, state-required form ​

•      Emergency Care Plan: required for students with allergies (food or otherwise) that could require medical attention

•      Over the Counter Authorization: permission (or declination) for OTC medication to be administered by school staff

•      For My Teacher: this is given to the teachers to help them get to know your child better

In addition, you will need to submit the following documents during the enrollment process:

•      Copy of the child's birth certificate

•      Current shots / Immunization records/ Immunization Exemption Affidavit

•      Signed wellness statement by child's physician (included in the enrollment packet)
